Quick Start Program—For Newly Prescribed Patients
Syndax understands that patients need access to treatments as soon as possible. SyndAccess may provide access to medication while the patient’s insurance is being authorized or reviewed. Complete the Quick Start section of the patient enrollment form to apply.
Eligibility criteria and terms and conditions:
- Provides a 15-day supply of Syndax medication until the prior authorization request is approved (not exceeding 2 shipments)
- Requires valid prescription in accordance with the Prescribing Information
- Patient must have any commercial insurance plan or a government program, and patient is actively pursuing initial coverage for Syndax medication
- Patient’s prescriber determines there is a medical necessity for the patient to start treatment immediately when facing an insurance delay of at least five (5) calendar days
Bridge Program—For Patients on Medication
Syndax cares about patients staying on the medication they need. SyndAccess can help patients who have had a lapse in insurance or gap while switching to a new plan. Complete the patient enrollment form to request access to free medication for your patient.
Eligibility criteria and terms and conditions:
- A one-time, 15-day supply of Syndax medication for eligible patients who are currently on treatment who experience an unexpected disruption in medication
- Patients may be eligible to receive an additional 15-day supply if the actual or anticipated disruption in insurance coverage will cause an interruption in treatment exceeding five (5) calendar days
- Following the patient’s transfer to SyndAccess, their specialty pharmacy provider shall continue to seek prior authorization from the patient’s health plan and coordinate with SyndAccess as necessary
